The Aged Care workstream has been meeting since August 2009 and has had direct input into the design of aged care services in Canterbury.

Aged Care Workstream Goal

To keep older people at home as long as possible, contributing to the aspirational targets of:

  • 15% reduction in rest home admissions
  • 6-12 months' extension of independent living for those people who eventually need rest home care
  • Contributing to the lower ED and acute admission targets proposed by the Urgent Care initiatives.
Key Initiatives to Achieve the Goal 
  • Use the development of integrated health and social services clusters to:
    • Align Older Persons Health Specialist Services (OPHSS) community team members, community nurses, pharmacists, physiotherapists and other clinicians as appropriate for the benefit of the individual patient and their carers.
    • Identify relevent social services and link to each cluster.
    • Align care workers with each cluster, based on requirements identified through model of care analysis.
  • Complete the restorative home support implementation trial and roll out the final model across Canterbury.
  • Improve integration of primary and secondary services through appointment of liaison clinicians, community multi-disciplinary post-discharge services, pharmacist medication reviews and telemedicine.
