Aged Care Workstream Goal
To keep older people at home as long as possible, contributing to the aspirational targets of:
- 15% reduction in rest home admissions
- 6-12 months' extension of independent living for those people who eventually need rest home care
- Contributing to the lower ED and acute admission targets proposed by the Urgent Care initiatives.
Key Initiatives to Achieve the Goal
- Use the development of integrated health and social services clusters to:
- Align Older Persons Health Specialist Services (OPHSS) community team members, community nurses, pharmacists, physiotherapists and other clinicians as appropriate for the benefit of the individual patient and their carers.
- Identify relevent social services and link to each cluster.
- Align care workers with each cluster, based on requirements identified through model of care analysis.
- Complete the restorative home support implementation trial and roll out the final model across Canterbury.
- Improve integration of primary and secondary services through appointment of liaison clinicians, community multi-disciplinary post-discharge services, pharmacist medication reviews and telemedicine.
