The Medication Management Service is a new service for Canterbury community pharmacy and general practice teams. The service's vision is to:

• Support patients to improve self-management of their medications to optimise the use and reduce the risk of harm of medication, reduce medication wastage and reduce the need for medicine oversight services.
• Improve relationships and service cohesiveness by developing an integrated whole of system approach to medication management services, wrapping multidisciplinary services around the patient for improved patient outcomes.
• Increase coverage of Medication Management Services in all Canterbury communities to meet referral demands (including from GPs, Secondary Care and Medicine Oversight) and Business Case deliverables, and to ensure access to people throughout the Canterbury region.
• Release resources to support people in the community, delay entry to residential care and reduce presentations to ED and AMAU.
• Encourage the maintenance of links between patients and their usual primary care providers.
