Acute Demand Acute Demand is a suite of community and primary care based services working together to keep people out of hospital. All age groups access these services.
C4 Canterbury Care Coordination Centre. Coordinates acute demand services, manages referrals for all DHB-funded short term services including district nursing and acts as the NASC (needs assessment and service coordination) for the over 65, non-complex client group.
CCPG Canterbury Community Pharmacy Group - a membership organisation for community pharmacists.
CCMS Collaborative Care Management Solution, the software developed by HSAGlobal for Canterbury to enable multi-disciplinary teams to deliver care for patients with high health needs.
CDHB Canterbury District Health Board.
Concerto Web-based portal used to view patient events, results, letters, etc.. Largely used within the DHB, but wider access is being implemented.
Connected Health / Health Network A dedicated secure network for health providers, governed by nationally managed standards, that enables registered and authorised health providers to store and exchange data with high levels of security.
CREST Community Rehabilitation Enablement and Support Team, a CCN initiative from the Aged Care workstream that supports earlier discharge for patients from hospital back out to their home setting. Currently aimed at the 65+ age patient group. Plans to extend to admission avoidance by increasing home-based support are in progress.
EOI Expression of Interest - a response to a request for submissions.
ERMS Electronic Referral Management System - a Canterbury-wide, Pegasus Health-developed referral system.
ESCRV Electronic Shared Care Record View - a view-only health record for all Canterbrians that can be accessed by authorised clinicians.
HOMER A CDHB system used to manage patient demographics, admissions, transfers and discharges.
IPA Independent Practitioner Association; a membership organisation of general practitioners.
MMS Medication Management Service - a service from the Aged Care workstream and managed by the CCPG that supports the upskilling of community pharmacists to complete medication management reviews, improving patient understanding of medication-related outcomes.
PHO According to Wikipedia: Primary Health Organisations (PHOs) are a collection of health providers, which are funded on a capitation basis by the NZ government via its District Health Board. They are usually set up as not-for-profit trusts, and have as their goal the improvement of their population's health.
PMS Practice Management System - the software used in general practice to manage their practice. Predominantly Medtech-32 is used in Canterbury.
Primary Care The health care services delivered at the first point of contact to the patient, typically general practice, community pharmacy, community health providers, etc.
SAP The clinical IT system currently being used by Older Persons Health and Mental Health Services within the CDHB.
Secondary Care Clinicians working within the hospital.
Shared Care More than just a shared electronic health record, a shared care model implies a multi-disciplinary team effort supporting the care of patients, particularly those with greater or complex health needs. The patient is an important member of the care team and they will be asked to 'opt-in' a shared care model (rather than blanket enrolment for all patients), providing explicit agreement to sharing of important health information between specific health providers.
 SPOE  Single Point of Entry - where patients are commonly referred to enter the health system such as medical outpatients, community service teams, CREST, physio outpatients, mental health, community stroke, etc.
TONIQ One of the common community pharmacy IT systems.
